Description of adverse reaction: This spontaneous report as received from a Licensed Practical Nurse refers to a female patient of unknown age. The patient's medical history, concurrent condition and concomitant medications were not reported. On 10-JUN-2013, the patient was vaccinated with first dose of GARDASIL (Lot unknown, Expiry date unknown) for prophylaxis. The nurse reported the patient received her second dose of GARDASIL 9 (Lot unknown, Expiry date unknown) on 21-NOV-2016 at 19 years of age (inappropriate schedule of drug administration)(interchange of vaccine products) and received her third dose of GARDASIL 9 (Lot unknown, Expiry date unknown) on 25-JAN-2017 at 19 years of age.
